چکیده
Received 28 February 2014; accepted 4 March 2014 We herein present a 78-year-old man with a 3-day history of jaundice and fever with chills. His past medical history was unremarkable except for smoking. He presented clinically with jaundice, anemia, abdominal scratch marks and hypotension. Laboratory investigation revealed microcytic hypochromic anemia, leukocytosis and an elevated serum creatinine (1.8 mg/dL). Liver function tests were deranged with hyperbilirubinemia and raised alkaline phosphatase [total bilirubin 11.4 mg/dL (0.1-1.1); direct bilirubin 8.6 mg/dL (0.1-0.4); alkaline phosphatase 874 IU/L (30-140)]. All viral serology was normal and CA 19-9 was 156 U/mL (0-37). Abdominal ultrasonography showed a dilated common bile duct (10 mm) with abrupt distal tapering. MRI/MRCP were done which showed a mass lesion at ampulla with a proximally dilated common bile duct. Side viewing endoscopy for management of acute cholangitis was done (Fig. 1A). However a choledochoscopy with an ultrathin neonatal endoscope revealed a calculus and ductal clearance was done with balloon sweep (Fig. 1B). Histological examination of the biopsy was suggestive of ampullary adenocarcinoma. Acute ascending cholangitis may be an initial presentation of ampullary tumor and requires urgent biliary decompression with endoscopic biliary drainage [1]. Elderly patients with cholangitis also have a higher incidence of coexisting medical problems like cardiovascular, pulmonary, neurological and other systemic diseases. Due to the high incidence of severe cholangitis, hypotension, altered sensorium and associated other concurrent systemic diseases, the management of acute cholangitis in the elderly becomes difficult [2]. Biliopancreatic malignancy constitutes a common cause of biliary obstruction in elderly patients compared to benign diseases in younger patients. However a commoner etiology of acute suppurative cholangitis may be missed without choledochoscopy. Figure 1 (A) Ampullary tumor. (B) Choledochoscopy showing solitary choledocholithiasis B A

Establishment of a novel rat model of severe acute cholangitis
Objective(s):This study aimed to establish a novel non-binding, reversible rat model of acute cholangitis of the severe type (ACST). Materials and Methods:Twenty-six rats were randomly divided into the sham-operated group (n=13) and the ACST group (n=13). All rats were intubated with a modified catheter through the external jugular vein. The ACST model was established by ligation of the distal ...
متن کاملTherapeutic ERCP in patient with situs inversus totalis and ampullary diverticulum.
Situs Inversus Totalis (SIT) is a rare entity with complete transposition of all viscera, making endoscopic and surgical procedures challenging and complicated. We describe a rare case of a 55 years old man with SIT and ampullary diverticulum presenting with cholangitis and deranged liver function tests due to common bile duct stones. Therapeutic Endoscopic Retrograde Cholangiopancreatography (...
متن کاملRecurrent acute pancreatitis in a patient with wirsungocele and neuroendocrine tumor of ampulla of Vater.
CONTEXT Wirsungocele has recently been shown to be associated with acute recurrent, severe necrotizing pancreatitis and chronic pancreatitis or chronic pain in abdomen. Till to date there is no report on association of wirsungocele with an ampullary neuroendocrine tumor, and recurrent pancreatitis. CASE REPORT We report a first ever case of wirsungocele diagnosed on EUS, its association with ...
متن کاملA Unique Use of a Double-Pigtail Plastic Stent: Correction of Kinking of the Common Bile Duct Due to a Metal Stent
A 72-year-old man with jaundice by ampullary adenocarci-noma was treated at our hospital. For biliary decompression, a transpapillary, fully covered, self-expandable metal stent (FCSEMS) was deployed. Four days later, the patient devel-oped acute cholangitis. Endoscopic carbon dioxide cholan-giography revealed kinking of the common bile duct above the proximal end of the FCSEMS. A 7-F double-pi...
متن کاملEarly ERCP in acute gallstone pancreatitis without cholangitis: a meta-analysis.
CONTEXT Early ERCP was reported to result in recovery from acute gallstone pancreatitis. To date, several RCTs comparing it to conservative treatment have yielded different results. OBJECTIVE We conducted a meta-analysis to determine the effect of early ERCP on the morbidity and mortality of acute gallstone pancreatitis without cholangitis. METHODS We searched the following databases up to ...
